IMPORTANT INFORMATION, FORMS AND MAP SOURCES:

  • EMERGENCY MEDICAL TREATMENT FORM: Go here for the form: http://www.aylc.org/forms.htm or click HERE.

IMPORTANT! If your child was enrolled as a Spring Competitive season player, and completed an Emergency Medical Treatment form, that form DOES NOT APPLY to Fall Clinic attendance. You MUST fill out a new Emergency Medical Treatment form for the Fall Clinic sessions, and make sure that AYLC receives it.

Please print or type all information on the Emergency Medical Treatment Form legibly. Your child's welfare might depend on our ability to read this information, and its thoroughness and accuracy. Pay careful attention to Sections 11, 12 and 13 in the forthcoming Event Details area of the ACTIVE.COM Online Registration process.

  • EMERGENCY CONTACT INFORMATION: Many parents put their own name and phone number in the Emergency Contact field. This DOESN'T HELP AT ALL! In an emergency situation we NEED A REAL, ALTERNATE PERSON to contact if we cannot reach the parent or guardian. A relative, a friend or a neighbor; someone should be willing to help you with this requirement.

  • REFUNDS will be issued sometime in October, less a $25.00 online processing fee, if AYLC is notified of withdrawal, in writing, prior to September 21, 2009 via a letter to: (AYLC, Inc., PO Box 5200, Arlington, VA 22205) No refunds permitted thereafter.

  • EQUIPMENT: Each child MUST be present for proper fitting during equipment distribution times. Girls' sticks and goggles available requiring $50.00 security deposit each. Girls must supply mouth guard. Boys' shoulder and arm pads, helmets and gloves are available, requiring $150.00 security deposit. Boys must supply protective cup, stick and mouth guard. Deposit checks refunded upon return of the items in good condition at the end of the last clinic session.
(AYLC sells balls for $2.00, mouth guards for $2.00, girl's goggles for $35.00, and boy's and girl's sticks for $32.00.)
  • IMPORTANT! During the online registration process you will be asked to state what equipment you wish AYLC to provide for your child's participation. Please be accurate in your response to this question so that we are able to acquire sufficient inventory to cover our need at least one week before the first clinic session begins. Our equipment supply is limited so priority during equipment distribution will be given to those who register earliest.

Our limited supply of loaner equipment will be distributed in order of registration date. A list showing your place in line will be emailed to those who requested equipment before the first equipment distribution date.

If you don't request equipment during registration, and you appear during equipment distribution time, YOU MUST WAIT until all of the equipment has been issued to those who did respond to our request before we can accommodate you.

There are no discounts or reductions on equipment deposits. Please bring your checkbook to equipment distribution signout. Make checks payable to AYLC, Inc.

CREDIT CARD PAYMENTS CANNOT BE ACCEPTED for equipment deposits.
  • GIRLS - EQUIPMENT SIGN OUT: ( stick and goggles ) Saturday, September 12th.

  • BOYS - EQUIPMENT SIGN OUT: Saturday, September 12th and Sunday, September 13th.

  • Each sign out session will be from 8:00 A.M. to 11:00 A.M. at the AYLC equipment trailer, rain or shine. (The trailer is located just north of Route 50 where N. Carlin Springs Road and N. Kensington Street meet. There is a small gravel road that leads about 100 yards back to the electric power station. The AYLC trailer is located near the power station.) EMAIL CONSIDERATIONS

    ALTERNATE EMAIL ADDRESS

To increase our chances of getting information to clinic participants, please, if possible, include a second NON-WORK RELATED email address where you can be contacted. Since clinic emails may need to be sent on Fri. or Sat.; if you only supply a work email address you will possibly not receive important information on updates or changes.

FREE EMAIL ACCOUNT - GET ONE - WHY?

ALL of the major local ISP's are blocking our email messages, and those of other NVYLL clubs too. (AOL, Verizon, Comcast, maybe Cox) See: "MORE INFO ON THE EMAIL "BLOCKING" PROBLEM" at http://www.aylc.org/sitenews2008.htm for additional information about this problem.

FREE email service is provided by Yahoo Mail, Gmail, Hotmail and others. In order to insure receiving information from us, please create an alternate address with one of these services and tell us about your new address with an ADD ME email request to AYLC. See how to ADD YOUR EMAIL ADDRESS on our website at: http://www.aylc.org/faq.htm

EMAIL ADDRESS CHANGES - NOTIFY US

If at any time during the year your email address should change, please let us know. The easiest way is to send a REMOVE ME request for the old address, and then an ADD ME request for the new address. Please include your name somewhere in the emails for accurate identification. and TELL US IF YOU ARE AN ARLINGTON RESIDENT. See the Email FAQ section of our FAQ page for more information on email addresses.
