AUTHORIZATION FOR EMERGENCY MEDICAL TREATMENT FORM for the next AYLC sponsored activity must be submitted and received by AYLC before a child is permitted to participate in any AYLC activity.

A small number of "HARD COPIES" of announcement flyers and Authorization For Emergency Medical Treatment Forms for AYLC sponsored camps, clinic and Spring Competitive Season will be available a few weeks prior to the start of each activity at:
- AYLC by calling 703-524-4529 (524-4LAX).
- Arlington County Community Centers, libraries, Public School Information Center and Arlington County swimming pools.
- Physical Education Departments in parochial, private and public elementary and middle schools within Arlington County. (High School Physical Education Departments only receive Fall Clinic announcement flyers and forms.)
