During the online registration process PLEASE use standard English language conventions when answering the questions on the form. Capitalize the first letter of all names, streets, etc. AND PLEASE, DO NOT USE ALL CAPITAL LETTERS when entering field information. Your cooperation here will save editing time on our part and help us produce better, more easily read reports for our coaches and commissioners. Thank you. When you've completed the registration process ACTIVE.COM gives you the opportunity to print a "hard paper copy" of your Registration Confirmation. Do this if you want to, but please see the note below for what AYLC really needs from you. There are two ways in which you can provide the REQUIRED DOCUMENTATION necessary to confirm a successful registration in our Spring Competitive Season. NOTE: You may choose METHOD ONE or METHOD TWO. You do not need to do both to fulfill the Confirmation of a Successful Registration requirement. METHOD ONE Right after you pay for your registration with your credit card, ACTIVE.COM presents a new screen in your browser that says "Your registration has been successful!" It looks like the sample below:  PRINT THIS PAGE !!! Notice that this printed browser page DOES NOT have your youngster's name anywhere on it, so ... write your youngster's name on the paper you just printed and bring it with you to equipment distribution or to the first Spring Competitive Season practice session, along with your completed Emergency Medical Treatment Form. Do NOT print the waiver unless you want a copy for your personal use. It WILL NOT serve to confirm a successful Spring Competitive Season registration. METHOD TWO After registration is completed, you will receive a number of confirmation email messages from ACTIVE.COM. One of these email messages will also serve to confirm your registration in the Spring Competitive Season. It will come from SUPPORT@ACTIVE.COM and the subject of the email is PAYMENT RECEIPT. It will look something like this, but not exactly, depending on your email program. FIND IT. Just make sure you have the email that has a SUBJECT line of PAYMENT RECEIPT. IF YOU DON'T SEE ANY EMAIL FROM ACTIVE.COM IN YOUR INBOX THEN CHECK THE "JUNK" OR "SPAM" FOLDER OF YOUR EMAIL PROGRAM. THEY MIGHT BE BEING RECEIVED BY YOUR EMAIL PROGRAM BUT FILED THERE . ( sample email message received from ACTIVE.COM )  ( sample email message received from ACTIVE.COM ) To print this email message, click on FILE (in the top left corner of your screen) and then PRINT in the drop-down menu that appears. Many web browsers will also print the page by pressing Ctrl-P ( hold down Ctrl key and press P while continuing to press Ctrl ). Notice that this printed email message DOES NOT have your youngster's name anywhere on it, so ... write your youngster's name on the email message you just printed and bring it with you to equipment distribution or to the first Spring Competitive Season practice session, along with your completed Emergency Medical Treatment Form. No equipment will be distributed to those who do not have the printed copy of this PAYMENT RECEIPT or a copy of the printed "REGISTRATION HAS BEEN SUCCESSFUL" PAGE. This hard copy printout will help us determine a successful registration in case of difficulties or "bugs" in the process, which we've had in the past. IMPORTANT: If you did not turn in your Authorization for Emergency Medical Treatment form and your Payment Receipt OR REGISTRATION HAS BEEN SUCCESSFUL page from ACTIVE.COM during equipment distribution please REMEMBER to bring both of these forms to the first Spring Competitive Season session. ARLINGTON COUNTY HOLD HARMELESS AGREEMENT You must agree with the terms of this clause to use Arlington County facilities. As consideration for the right to participate in the Arlington Youth Lacrosse Club Spring Competitive Season 2008 and/or other activities and services provided by the Arlington County Department of Parks, Recreation, and Cultural Resources, its agents and employees, I, on behalf of myself, my executors, administrators, heirs, next of kin, and successors, hereby covenant to hold harmless and indemnify the County and all of its officers, departments, agencies, agent and employees from any and all claims, losses, damages, injuries, fines, penalties and costs (including court cost and attorney's fees), charges, liabilities, or exposures, however caused, resulting from, arising out of, or in any connection to my or any minor family member's participation in the above described program. I have read and understand this INDEMNIFICATION AND HOLD HARMLESS AGREEMENT, and by my ELECTRONIC SIGNATURE at the end of the ACTIVE.COM registration process agree to its terms.
