NOTE: Please keep in mind, the requirement for completing the Authorization For Emergency Medical Treatment Form for EACH child you wish to enroll in an AYLC sponsored activity. Separate forms are required for participation in the Spring Competitive Season and / or for participation in the Fall Clinic. These are important so why not print them now so you don't forget about them. If it is not necessary for you to attend equipment distribution, please remember to bring this form to the first Spring Competitive Season practice session. If you registered within a few days of the first practice session, please BE SURE to BRING a copy of the required Emergency Medical Treatment Form with you to the practice session. If you have already mailed it, we might not have received it in time to allow the youngster to participate that day. So please print and complete another copy and bring it with you! This agreement also includes any individual acting in a volunteer capacity for the Arlington Youth Lacrosse Club, Inc.
